New patient referral and scheduling

Most insurances require a referral to see a rheumatologist. If your insurance requires a referral, please make sure your primary care or other referring doctor has sent it to our office prior to your appointment. If you receive your referral confirmation letter prior to your visit, please bring it with you to your appointment.

Please request your referring provider to send us all the relevant medical records (chart notes, labs, imaging tests, or any other records to help with your evaluation) ahead of your appointment. Without records your rheumatology specialist may not be able to provide a complete consultation.

After your doctor has sent us your referral, please make sure that you call us to schedule your appointment at 503-297-3384, Option 2. We are not able to call patients due to the volume of referrals received. We routinely notify referring offices of this policy as well.  If you need to be seen urgently please have your provider reach out to one of our providers to discuss your case at 503-297-3384.

Please note that we are not on EPIC or Mychart. We still need your referring physician to fax your relevant medical records at 503-297-0863.

Patients referred for infusion / injection treatments

We also offer infusion and injection treatments to patients referred by outside providers e.g prolia, reclast, evenity, remicade, rituxan, among others.

Referring Providers: Please check our infusion center page for the detailed process on how to refer a patient to us for infusion/ injection medication.
